when pressing play in the madexcept wizard
i see "UnNamed Assistance" as the form caption

how can this be changed?

The "play" button is just for testing. The title is normally set by the assistant's name, which you can edit in the assistant editor (top half of the settings dialog).

You're testing forms only (bottom half of the settings dialog). Press the other "play" button in the assistant category (top half of the settings dialog).
